DeVry University and its Keller Graduate School of Management students and alumni:
Students please download and complete the transcript request form (PDF) and send it to the appropriate fax number or mailing address of the location you are currently attending or the last location you attended. The form provides a drop down selection list in the "Last Location Attended" field from which you can select the appropriate location and populate the form with the correct address, fax, and phone number for the location selected. Please submit the form to the location selected. Failure to complete exit counseling may result in placement of a hold on students' records, which would prevent fulfillment of transcript requests and release of graduates' diplomas. Diplomas and transcripts are not released to students with outstanding balances on their student accounts. Official transcripts are not issued until all financial obligations to any DeVry institution are fulfilled.
• Conferrals for graduate programs occur at the end of the session and can take up to three (3) weeks.
• If you would like your transcript held for final term GPA and/or conferral, please indicate this preference in your request.
